Trauma Trainer

Timed decision drills for improvised emergency care. The premise: you are the person who is there — no trauma kit, no equipment, and emergency services minutes to an hour away. What you rehearse is deciding under that clock.

Nothing in this trainer is clinically validated. Every scenario ships marked draft-unvalidated: the clinical content has not been reviewed by a medical professional of any kind.

This is a training simulator, not medical guidance — not a first-aid course and not a substitute for one. Its vitals and timings are illustrative. Do not use it to treat a real injury.

In a real emergency, call your local emergency number.

What it is

9 playable scenarios, each a timed decision sequence run against a live physiology model: a simulated bleed that keeps running while you decide. Spoken coaching lines (81 recorded cues) narrate the run, and a debrief scores it and traces every choice you made. It runs in the browser and installs as an offline-capable app.

How a run plays

  1. The brief. A dispatch-style picture of the scene, with the EMS arrival time stated up front.
  2. Decisions on the clock. From the first step, the simulated bleed runs — and keeps running until you act to control it. Hesitation has a simulated cost you can watch.
  3. Myth gates. An option built on a common first-aid myth doesn't just get marked wrong. The run pauses and you must pick why it fails before you can continue — in the trainer's own words, "lock in the reasoning; that's what holds under stress."
  4. The hold. Getting the bleeding controlled is not the end. While the crew closes in — live countdown, proximity map — you reassess: is it still working, are they deteriorating, does anything need redoing. The dead time between checks is compressed in labelled jumps; the checks themselves run in real time, and a missed one is itself a decision.
  5. Handover. The run ends when EMS arrives, and you are scored on the state your patient is in at that moment. In one scenario, deliberately, the drill ends before anyone arrives — "help is not coming soon, and whatever you do has to hold without it."
  6. The debrief. Score against your best, the one thing to fix next run, your full choice list, and a downloadable run log.

What the training gives you

These are benefits of the training — rehearsal, pressure and feedback — not claims about real-world outcomes. Nothing here has been validated.

The scenarios

All 9, quoted from the trainer's own scenario files — titles, framing and dispatch briefs verbatim. Every one is draft, unvalidated.

Massive limb hemorrhage

improvised tourniquet · the patient is bleeding while you decide

Roadside crash · arterial bleed, mid-thigh · EMS 5 min (3 km out) · no trauma kit. Five minutes is longer than this patient has — the clock runs until you get a tourniquet tight.

EMS 5 min · 3 kmdraft-unvalidated

Junctional hemorrhage

high groin bleed · wound packing · when a tourniquet is the WRONG answer

Stabbing · deep wound high in the groin crease, arterial bleeding · EMS 4 min (3 km out) · no trauma kit. Four minutes of holding, and this bleed is too high on the limb for any tourniquet to sit above it.

EMS 4 min · 3 kmdraft-unvalidated

Open femur fracture

heavy bleed + broken thigh · control first, then improvise a splint

Fall from height · open mid-thigh fracture, bone ends visible, the leg is deformed and bleeding heavily · EMS 9 min (6 km out) · no trauma kit. A femur can lose a lot of blood in nine minutes — and the order you work in matters.

EMS 9 min · 6 kmdraft-unvalidated

Impaled object

object stuck in the wound · leave it in, bleed control around it

Workshop accident · a metal rod is impaled deep in the outer thigh, still in place, bleeding around it · EMS 6 min (4 km out) · no trauma kit. What you do with the object in those six minutes decides whether this bleed gets better or catastrophically worse.

EMS 6 min · 4 kmdraft-unvalidated

School bus collision

pediatric entrapment · parents on scene · difficulty: hard

School bus collision · a 9-year-old pinned by a deformed seat, arterial bleeding from the lower leg, pale and frightened · traffic still moving · parents arriving · EMS 7 min (5 km out). A child has less blood to lose — seven minutes is not the margin it sounds like.

EMS 7 min · 5 kmdraft-unvalidated

Penetrating torso trauma

assault behind a locked gate · controlled entry · pressure, not packing · difficulty: hard

Correctional facility assault · an adult stabbed in the side of the chest, bleeding heavily, pale and struggling to speak · officers control your entry until the wing is secure · EMS 6 min (4 km out). You can't reach him yet — and once you can, every second counts.

EMS 6 min · 4 kmdraft-unvalidated

Stairwell femur bleed

open femur, heavy hemorrhage · put your bystanders to work · difficulty: moderate

Office stairwell fall · an adult with an open mid-thigh fracture, bright red blood running down the steps, pale and sweating · co-workers crowding, unsure · EMS 8 min (5 km out). You have hands available — use them, or lose the minutes they could save.

EMS 8 min · 5 kmdraft-unvalidated

Arterial bleed, panicking partner

glass laceration, spurting forearm · one patient, two problems · difficulty: moderate

Domestic scene — an adult put an arm through a glass door · deep inner-forearm laceration, blood spurting, patient pale and anxious · partner panicking and escalating, not violent · EMS 4 min (3 km out). Two problems in the room; only one of them is bleeding.

EMS 4 min · 3 kmdraft-unvalidated

Prolonged field care

chainsaw wound, 50 minutes from help · the tourniquet is only the start · difficulty: hard

Remote woodlot · chainsaw kickback, deep outer-thigh laceration, arterial, the patient pale and shaky · one bar of signal, cold afternoon · you called it in eight minutes ago and the crew is still 50 min (38 km) out. These next few minutes are an excerpt of that wait, not the whole of it — help is not coming soon, and whatever you do has to hold without it.
